Licensed Practical Nurse

Meridian Center - NC in Burlington seeks a compassionate Licensed Practical Nurse to deliver resident-centered care in a skilled nursing and rehab setting. The LPN will provide hands-on clinical care, administer medications, monitor conditions, and support recovery and long-term quality of life for seniors and medically complex patients. Working within an interdisciplinary team, you'll collaborate with RNs, therapy, and support staff while documenting in the EHR, upholding infection control and safety standards, and communicating with residents and families.

Responsibilities

• Provide direct nursing care to residents, including treatments and hygiene support
• Administer medications and treatments per physician orders and facility protocols
• Monitor vital signs, observe residents' conditions, and report changes to RN or provider
• Document care, assessments, and interventions accurately in the EHRParticipate in interdisciplinary care planning and family communication
• Support safe transitions between rehab, long-term care, and other levels of care
• Maintain infection control and resident safety standards at all times
• Mentor and direct CNAs to ensure quality, resident-centered care
