Float Pool Licensed Practical Nurse

Meridian Center - NC in Burlington is seeking a Float Pool Licensed Practical Nurse to provide high-quality bedside care across multiple units in a skilled nursing and rehab setting. The LPN will assess residents, administer medications and treatments, monitor changes in condition, and collaborate with RNs, physicians, and therapists to support care plans. This role flexes between short-term rehab and long-term care units based on census and acuity.

The ideal candidate is dependable, compassionate, comfortable with changing assignments, and committed to resident dignity, safety, and strong communication with families and the care team.

Responsibilities

• Provide direct nursing care to residents across multiple units based on staffing needs
• Administer medications and treatments per physician orders and facility protocols
• Monitor, document, and report changes in resident condition to RNs and providers
• Support and implement individualized care plans for short-term rehab and long-term care residents
• Perform wound care, catheter care, and other LPN-level clinical procedures
• Collaborate with interdisciplinary team members, including therapy and support staff
• Educate residents and families on care plans, medications, and health status
• Maintain accurate, timely documentation in the electronic medical record
• Adhere to infection control, safety, and regulatory standards
• Support a resident-centered culture of dignity, compassion, and respect
