Responsibilities:Provide effective, equitable value-based primary care to adult Medicare patients, including routine and acute office visits, wellness exams, care coordination, health education, and participation in population health initiatives. Collaborate with a multidisciplinary team, conduct home visits if needed, and assist with phone triage and outreach. Focus on building trusting patient relationships and ensuring comprehensive documentation. Work closely with operational leadership and the care team to deliver high-quality care.
Qualifications:- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate
- National certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner
- Active, non-probationary state NP license and DEA license
- US Work Authorization
Skills:
Experience in primary care, understanding of HCC documentation, ICD-10 coding, and health risk assessments; teamwork-oriented; bilingual skills are a plus.
