What You'll Do

In this dynamic role, you will work closely with youth in our HEARTH Beyond Program and provide support at My Sister Susan's House, our residential facility for youth. Your main responsibility will be to create and implement individualized case management plans to ensure successful outcomes for our clients. You will engage with clients face-to-face, coordinate evaluations, and collaborate with them to develop client-centered plans focused on health, wellness, and personal development.

Qualifications

Education and Experience
  • Minimum age of 21 years.
  • Bachelor's degree from a four-year college or university, with at least two years' experience providing case management, or equivalent combination of education and experience.
  • Proficient in reading and interpreting documents, as well as writing routine reports and correspondence.
  • Basic math skills, including multiplication, division, understanding units of measure, and using whole numbers, fractions, and decimals.
  • Ability to follow written, oral, or diagram form instructions with common sense understanding.
  • Valid driver's license, at least three years of driving experience, and a driving record in good standing.
