The Care Team Hospice is seeking a compassionate and experienced Nurse Practitioner to join our growing team!!. This is an excellent opportunity to make a meaningful difference by providing expert clinical assessments, symptom management, and compassionate end-of-life care for hospice patients in their homes and care facilities.
As a Hospice Nurse Practitioner, you'll work alongside physicians, nurses, social workers, and other members of our interdisciplinary team to ensure patients receive exceptional, patient-centered care. A primary focus of this role is completing required Face-to-Face (F2F) hospice recertification visits while supporting patients and families through every stage of their hospice journey.

Compensation- $130 per completed Face-to-Face (F2F) Visit/Per Visit

Key Responsibilities
- Complete timely Face-to-Face (F2F) encounters for hospice recertification in accordance with Medicare guidelines.
- Perform comprehensive clinical assessments, evaluate disease progression, and communicate findings to the Hospice Medical Director and interdisciplinary team.
- Collaborate with physicians, nurses, and other care team members to develop and support individualized plans of care.
- Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and end-of-life care.
- Maintain accurate and timely documentation of all patient encounters while ensuring compliance with hospice regulations and agency policies.
- Current Nurse Practitioner (APRN/NP) license in the state of Indiana.
- Graduate of an accredited Nurse Practitioner program.
- Minimum of two years of advanced practice or clinical nursing experience; hospice, palliative care, home health, geriatrics, or primary care experience preferred.
- Strong clinical assessment, communication, and critical thinking skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Knowledge of hospice philosophy, symptom management, and Medicare hospice regulations preferred.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
- Current CPR certification.
- Current TB testing in accordance with agency policy.
