The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison (CCL) to grow our presence in the community. - Build and expand relationships with physicians, healthcare partners, and key decision‑makers to drive referral volume and market growth.
- Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort‑focused, patient‑centered care.
- Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community.
- Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well‑being.
- Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.
- Education: Bachelor’s degree or equivalent.
- Experience: 1‑2 years healthcare specific outside sales experience preferred; 1‑2 years outside sales experience preferred or considerable transferrable experience.
- Ability to perform regular travel within assigned geography and extended geography for training and meetings.
- Must have reliable transportation; valid and current driver’s license and current auto insurance required.
