Nurse Practitioner

Salary: $110,000 - $140,000 per year

• Deliver care within senior living and post-acute communities
• Diagnose and manage acute, episodic, and chronic conditions
• Order and interpret diagnostic testing
• Prescribe medications as appropriate
• Develop and manage patient care plans
• Collaborate with interdisciplinary care teams
• Maintain accurate, timely documentation
• Educate patients and caregivers on disease management

• Active Illinois NP or PA license
• Minimum 6 months of NP/PA experience
• Reliable, insured transportation
• Strong clinical judgment and independent decision-making skills

• 2+ years in long-term care or skilled nursing
• Experience managing cardiac or complex chronic conditions
