Journey Care Hospice in Highland Park, IL is seeking a compassionate Hospice RN Case Manager to deliver patient-centered, end-of-life care. You will manage a dedicated caseload with interdisciplinary support and coordinate with physicians to create personalized care plans.
Work schedule is Monday–Friday, 8:30 AM–5:00 PM with no on-call. Salary ranges from $80,000 to $97,000 per year, plus a comprehensive benefits package and enrollment in company programs.
