Journey Care Hospice in Highland Park, IL is seeking a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care. You will manage a dedicated caseload and work with an interdisciplinary team to ensure comfort and dignity.
Schedule is Monday–Friday, 8:30 AM–5:00 PM with no on-call. Salary ranges from $80,000 to $97,000 based on experience, plus 401(k), medical/dental/vision, generous PTO, holidays, and certification support.
