Prairie View in Newton, KS is seeking an Adult Behavioral Health Case Manager to advocate for vulnerable adults and coordinate access to housing, medical care, and community resources.
You will work with adults, older adults, caregivers, and partners (including DCF) to reduce risk and improve quality of life, using a strengths-based approach within an interdisciplinary ACT team setting, with emphasis on person-centered planning, timely documentation, and collaborative care.
