• Active and unrestricted Physical Therapist (PT) or Occupational Therapist (OT) license in the state of Ohio required
• Graduate of an accredited PT or OT program applicable to discipline
• Previous therapy experience preferred
• Home Health experience preferred
• Ability to evaluate patients and develop individualized plans of care
• Strong clinical assessment, communication, and documentation skills
• Ability to work independently in a home-based care environment
• Valid driver's license and reliable transportation required
• Ability to travel locally to patient homes throughout the assigned service area
• Ability to perform the physical duties associated with the applicable therapy discipline and patient care
• Ability to pass background screening and onboarding requirements

• Complete patient evaluations and determine appropriate therapy needs based on discipline
• Develop and implement individualized plans of care
• Provide skilled physical or occupational therapy services according to the patient's plan of care
• Establish measurable goals focused on improving function, safety, mobility, independence, and quality of life
• Evaluate patient response to treatment and modify the plan of care as clinically appropriate
• Complete OASIS visits, evaluations, reassessments, discharge documentation, progress notes, and other required clinical records accurately and on time
• Educate patients, families, and caregivers regarding treatment plans, exercises, safety, adaptive techniques, and home care recommendations
• Communicate patient progress and changes in condition with physicians and members of the interdisciplinary care team
• Coordinate with and provide appropriate direction to Physical Therapist Assistants (PTAs) or Occupational Therapy Assistants (OTAs) involved in the patient's plan of care
• Promote patient safety, independence, and quality of life in the home environment
• Follow agency policies, clinical standards, HIPAA requirements, and applicable regulatory guidelines

Wise Home Care is seeking Physical Therapists (PTs) and Occupational Therapists (OTs) for PRN Home Health opportunities throughout Highland and Fayette Counties, Ohio. These are flexible, per-visit opportunities with compensation ranging from $120-$150 per visit based on discipline and visit type. Therapists will provide individualized services to patients in their homes, complete evaluations and assessments, develop and update individualized plans of care, provide skilled therapy interventions, educate patients and caregivers, coordinate with physicians and the interdisciplinary healthcare team, and maintain accurate and timely clinical documentation.

Service areas may include Hillsboro, Greenfield, Fayetteville, Bainbridge, Peebles, Washington Court House, Jeffersonville, Bloomingburg, Jamestown, Frankfort, and surrounding communities.
