RN Case Manager / Nursing Supervisor
Job SummaryAs an RN Case Manager / Nurse Supervisor, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes. With core skills in managed care, patient assessment, client care, treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. Your skills in critical care and infectious disease are valuable in handling complex cases, while your experience in home and community care and supervisory roles will enhance your ability to lead and support a multidisciplinary team.
You will ensure successful communication among physicians, agency staff, service administrators, caregivers, clinicians, and the client and their family.
- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
- Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document, communicate, and coordinate patient status changes to the care team.
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
- Maintain a minimum of one year of nursing supervisory experience; experience in home care preferred.
- At least 1 year of case management experience and home health agency (or equivalent) experience.
- Experience with OASIS and NON‑OASIS assessments.
- 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments (plus).
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN License and ability to practice in the State of Ohio.
- CPR and First Aide Certifications required (or can be obtained prior to hire date).
- 1 year experience in case management and home health required; 2+ years highly preferred.
- Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Strong attendance and dependability track record with previous employers.
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, accommodating, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Current Driver’s License and reliable transportation.
- Round‑trip / Door‑to‑Door mileage reimbursement.
- Autonomy in a positive, client‑centered culture with excellent Quality and Compliance support and training.
- No on‑call requirements / no overnight shifts.

Hourly Wage: $35 - $40/hour
LocationHillsboro, OH + surrounding areas in Southern Ohio. Primary service area is centralized in Highland County, with typical travel distances in a 10‑60 minute radius; occasional travel up to 90 minutes also required. Option for travel up to or beyond a 2‑hour radius is available if additional hours are desired (PT or FT), which may include Clermont, Hamilton, Clinton, Brown, Adams, and other counties.
ScheduleFlexible and customizable. Build a schedule as close to home as possible – fits your availability and preferences. Typical hours are Monday‑Friday during the day (8 am‑5 pm).
Employment TypePRN or Part‑Time (Long‑term employment). No on‑call requirements / no overnight shifts required. Weekend, overtime, respite, and PRN services offered when available but not required. Hourly or salary compensation (paid weekly on Fridays).
TravelWilling to travel surrounding counties. Primary travel required in 30‑60 minute daily radius (or less); up to 1‑2 hours (max.) required on occasion.
