Job Title
Home Infusion Clinical Liaison
Job DescriptionThe Home Infusion Clinical Liaison supports Oregon Home Infusion Pharmacy by building and maintaining relationships with discharge planners, facility staff, physicians and other stakeholders. The role assesses home infusion appropriateness, facilitates patient care transitions onto service, ensures documentation for ordered infusion therapy, and provides consultative services such as education and informational presentations on home infusion therapy for providers, staff, patients and families.
Responsibilities- Develop and maintain positive referral relationships to increase Home Infusion Referrals within assigned accounts
- Work with discharge planners, facility staff, physicians and other individuals to assess appropriateness of home infusion
- Facilitate patient care transitions onto service and ensure appropriate documentation
- Provide consultation and informational presentations regarding home infusion therapy
- Serve as a clinical resource and educator for ordering providers, facility staff, patients and families on Home Infusion Therapy
- Education sufficient for licensure
- Oregon Registered Nurse license upon hire
- National Provider BLS – American Heart Association certification upon hire
- Licensed in states where care is delivered at time of hire
- At least 1 year full‑time clinical experience
- Bachelor’s Degree in Nursing
- Liaison, marketing or sales experience
- Health care industry experience
- Documented prior successful sales/account medical management experience
- Epic EMR experience
