Registered Nurse (Home Health) - Full Time
Territory:
Kendall, Camp Douglas, Mauston, Hillsboro, and Elroy communities.
Sign-On Bonus: $10,000
What You’ll Do- Serve as the primary clinical partner for patients and families, delivering individualized, one-on-one nursing care in the home.
- Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into patient-centered care plans.
- Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management tailored to each patient’s goals.
- Educate and empower patients and caregivers through clear instruction, coaching, and support.
- Monitor patient progress closely, identify early changes, and communicate with physicians and interdisciplinary team to adjust plans and prevent avoidable hospitalizations.
- Coordinate care across providers and services to ensure continuity, clarity, and high-quality outcomes.
- Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes.

- Active Registered Nurse (RN) license in the state of practice.
- At least one (1) year of clinical nursing experience.
- Patient-focused, organized, and motivated by delivering high-quality care.
- Current driver’s license, reliable transportation, and auto insurance.
- CPR certified or able to obtain certification.
