Summary

Provide eligibility and enrollment assistance for QUEST/Medicaid, and Federal Marketplace health insurance to all community residents including uninsured patients on Maui, Molokai and Lanai. Serve high risk clients/families with a focus on the COFA community to assure early, ongoing preventive and follow-up care; to provide assistance to those with language barriers and those needing assistance with navigating through the health care and social service systems.

Position

Goal:

Provide kokua for accessing affordable health insurance services to the uninsured and underinsured Maui County population, naturally including the outer islands of Molokai and Lanai, while targeting COFA, migrant farm workers/service industry workers, homeless, IDUs, LGBTQ community, and recently released prison population. Seeking referrals, cohorts / collaboration with churches and community groups with access to hard to reach & at-risk communities.

In doing so we learn that there are families and children who could benefit from other services IMUA Family Services and our partner agencies whom have been overlooked because they have not been a part of the healthcare system in our community.

Essential Duties and Responsibilities:

Complete all training and turn in all paperwork required by DHS, MQD, and FFM.

Possess the ability to perform basic assessments and information gathering techniques.

Possess the ability to gather pertinent data and complete reports as needed and in a timely manner.

Ability to assess and make determinations concerning eligibility criteria for Medicaid, QUEST, Health Care.

Gov (Obama Care) and other private/non-profit/government programs.

Attend meetings as required by HIHAF and MQD, providing timely and informed responses when operational and/or administrative issues or questions arise.

Work well with staff and providers.

Provide prompt assistance to MQD staff regarding operational issues.

Assure timely compliance with all contract responsibilities.

Travel to consumers in communities and provide the primary role of assisting people by providing information on affordable health insurance and direct assistance through the enrollment process.

Build relationships with other community organizations.

Arrange and schedule outreaches in the community for targeted populations through health fairs, individual outreach, etc.

Provide feedback to Program Director on strategies for effective engagement with community members and the type of appropriate tools and measurable amounts for educating specific target populations.

Track and record data of numbers of consumers reached through outreach events for COFA, Homeless Population, Farm and Service Workers, LGBTQ Community, Newly Released Prisoners, General Population, Language Assistance Services for persons with Limited English Proficiency. Education activities, consumers reached, consumers served, collateral distributed, consumer support, enrollments for both KOLEA and Health Care.

Gov, 1095A Tax Referral calls/conversations. Including but not Limited to presentations, community events and health fairs, the types and numbers of other consumers’ assistance provided and number of people enrolled.

Submit data monthly to Healthcare Navigator Program Director for tabulation and reporting in a timely manner.

Submit mileage and expense reports monthly to the Manager.

Carry out and complete all other duties as assigned by the Manager.

Adjust work schedule when necessary to meet program needs.

Abide by all policies set forth in the employee manual.

Participate in the fundraising activities of IMUA Family Services.

May require some supervisory duties of other Community Workers/HC Navigator Staff/Volunteers/Contract Staff
• Perform other duties as assigned.
