American Traveler is seeking a Physical Therapist with at least 1 year of home health experience for a 13-week travel assignment serving a geriatric patient population across the Hawaiian Islands.

Job Details

• Home health setting serving a primarily geriatric patient population on the Big Island of Hawai'i,

• Day shift schedule, 5x8-hour shifts (8:00 AM - 4:30 PM),

• Productivity requirement of 25 units per week,

• Occasional floating to cover Kauai, Maui, and/or Oahu may be required; all float-related expenses are covered,

• Patient population includes those with neurological, orthopedic, cardiopulmonary, and cognitive conditions such as post-stroke, Parkinson's disease, dementia, COPD, CHF, and post-op surgical cases,

• Documentation via Matrix Care,

• Procedures and assessments include OASIS, plan of care development, reassessments, gait training, ROM exercises, strength testing, and discharge planning,

Job Requirements

• Minimum 1 year of recent home health PT experience required,

• Current BLS certification (AHA) required,

• Active Hawaii PT license required if applicable,

• Proficiency with geriatric, neurological, and orthopedic patient populations required,

• Experience with OASIS documentation and home health plan of care required,

• Familiarity with adaptive and assistive devices, including walkers, Hoyer lifts, wheelchairs, and lower extremity prosthetics,

• Skills in pain assessment, vital signs monitoring, skin integrity evaluation, and patient/family education required,

• Experience with cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD) required,

• Strong time management skills required,

Additional Information

• Responsibilities include evaluation, treatment, goal-setting, home exercise program development, reassessments, and discharge planning,

• Patient and family education is a key component of the role, including safety, injury prevention, and identifying signs of abuse,

• Preferred experience includes wound care, lymphedema management, multiple sclerosis, spinal cord injury, traumatic brain injury, and neurodevelopmental techniques (NDT),

• Scrubs in any color and closed-toed shoes are required,
