Position overview The Physical Therapist provides comprehensive acute and outpatient rehabilitation services across hospital-based settings in a rural community environment. This role is best suited to an experienced, highly independent clinician who can manage a diverse caseload spanning inpatient, ICU, ER, SNF, Med/Surg, and outpatient clinic care.
Schedule Monday Friday, 8:00 AM 4:30 PM (lunch 12:00 1:00 PM).
Must be able to work Saturdays; no exceptions.
Key responsibilities Perform PT evaluations and treatments across inpatient (approximately 25%) and outpatient (approximately 75%) settings, including ICU, ER, Med/Surg, SNF, and a hospital-based outpatient clinic.
Manage mobility progression from acute admission through discharge planning, including gait training, transfers, fall prevention, and equipment recommendations.
Treat a wide range of outpatient diagnoses (orthopedic, neurologic, post-surgical, and chronic conditions) using independent clinical judgment and evidence-based practice.
Maintain a full, mixed caseload of approximately 5 15 patients per day, balancing acute and outpatient responsibilities and optimizing schedule efficiency.
Document evaluations, daily treatments, progress notes, and discharge summaries accurately and in a timely manner in the electronic health record.
Collaborate with physicians, nursing, case management, and other rehab professionals to support coordinated, patient-centered care and safe transitions.
Required qualifications & skills Active (or quickly obtainable) Hawaii Physical Therapist license.
Minimum 2 years of Physical Therapist experience, including both acute care and outpatient settings.
Current BLS/BCLS certification.
Demonstrated independence in managing complex, mixed-setting caseloads and making sound clinical decisions with minimal supervision.
Ability to work in a rural hospital environment, adapt to variable resources, and build rapport within a close-knit community.
Core competencies Acute and outpatient physical therapy evaluation and intervention.
Patient mobility, functional progression, and discharge planning from hospital to home or post-acute care.
Time management and organization in a high-mix, moderate-volume setting (5 15 patients/day).
Interdisciplinary collaboration across inpatient and outpatient teams.
Clinical independence, adaptability, and problem-solving in a rural healthcare context
