Physical Therapist (Home Health) in HILO, Hawaii. This position involves providing rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The therapist will perform evaluations, develop and implement personalized treatment plans, and deliver evidence-based therapeutic interventions to enhance residents' function and quality of life. The role requires collaboration with an interdisciplinary team and supervisory responsibilities when applicable.

  Rotating shifts. Typically scheduled for five days per week, with the possibility of including one weekend day.
• Required Qualifications:
  
  Current State of Hawaii license in good standing appropriate to the discipline, Basic Life Support (BLS) certification, and minimum of two (2) years of experience in the SNF/ICF setting preferred. Less than two (2) years of experience is acceptable.
• Preferred Qualifications:
  
  Experience in rehabilitation services within SNF/ICF settings, familiarity with electronic medical records such as Net Health Rehab, and ability to supervise therapy assistants.
• Additional Information:
  Candidate must arrive the Thursday before the start date to attend employee health appointment on Friday. No facility, hospital, clinic, organization, or employer names are included to maintain anonymity.
