The Physical Therapy Assistant (PTA) will provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Responsibilities include performing evaluations, implementing treatment plans, educating patients and families, and completing documentation.

• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).
• Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
• Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team.
• Implement evidence-based therapeutic interventions to maximize function, independence, mobility, and quality of life.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation of progress, changes in the plan of care, and discharges.
• Supervise therapy assistants, when applicable to the role.
• Be available to work Hawaii State and Federal Holidays as necessary.

• Generally scheduled five (5) days/week.
• Likely to include one weekend day.

• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license appropriate to the discipline in good standing.
• Any traveler not currently in possession of an active State of HI license must apply upon acceptance of offer. Candidate must have Hawaii License by start of assignment.

• Minimum two (2) years of experience in the SNF/ICF setting preferred.

• Orientation to Net Health Rehab Optima and Epic EMR documentation provided.

• Possible floating to other units or facilities based on census and staffing needs.
