Rehabilitation Therapist – Skilled Nursing Facility

Schedule:

Variable - 7 days per week departmental coverage - 5 day work week. Day shift.

Requirements:

• Current State of Hawaii license in good standing appropriate to the discipline
• Current Basic Life Support (BLS) certification
• Minimum two (2) years of experience in the Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF) setting preferred

Roles & Responsibilities:

• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).
• Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
• Implement evidence-based therapeutic interventions to maximize function and quality of life for patients.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation.

Compliance / Notes:

Must arrive the Thursday before start to attend the employee health appointment on Friday.
