Job Description:

Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) as a Physical Therapist. Responsibilities include performing evaluations, reassessments, and treatments consistent with licensure, discipline, and scope of practice.

Responsibilities:

• Perform evaluations, reassessments, treatments, and other clinical services.
• Develop and implement individualized treatment plans in collaboration with supervising therapist, physician, and team.
• Implement evidence-based therapeutic interventions.
• Educate residents, families, and caregivers on treatment plans and progress.
• Complete accurate documentation according to facility, state, federal, and payer requirements.
• Supervise therapy assistants when applicable.

Requirements:

• Minimum two (2) years of experience in the SNF/ICF setting preferred.
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline.
