Job Description:

This position is for a candidate with primary ACUTE Inpatient experience, primarily covering in ACUTE REHAB and secondarily in ICF/SNF Rehab. The role involves evaluating and re-evaluating assigned residents/patients; planning and implementing an ongoing therapeutic physical therapy program as prescribed by attending physicians and/or consulting specialists, and providing consulting physical therapist services. Must be available to work shifts, weekends, and holidays as necessary based on hospital operations.

Responsibilities:

• Evaluate and re-evaluate assigned residents/patients.
• Plan and implement an ongoing therapeutic physical therapy program.
• Provide consulting physical therapist services.

Requirements:

• Minimum 6 months work experience in at least one of the following settings working with adults:
  Acute, rehab hospital, SNF.
• Active State of HI license or must apply upon acceptance of offer; must have Hawaii License by start of assignment.
• BLS certification.
