Requirements:
Must possess an active State of HI physical therapy license by the start of the assignment. Minimum 6 months of work experience in at least one of the following settings: acute care, rehabilitation hospital, or skilled nursing facility (SNF). Basic Life Support (BLS) certification required. Experience using EMR software, specifically Meditech.
Roles & Responsibilities:
Evaluate and re-evaluate assigned residents/patients. Plan and implement ongoing therapeutic physical therapy programs as prescribed by the attending physician or consulting specialist. Provide consulting physical therapist services as needed. Maintain documentation of patient progress and communicate effectively with the healthcare team. Be available to work shifts, weekends, and holidays based on hospital operations.
Compliance / Notes:
Must arrive the Thursday before the start date to attend an employee health appointment on Friday.
