Variable - 7 days per week departmental coverage, 5 day work week. Day shift. Generally scheduled 5 days/week with the possibility of including one weekend day.

Current State of Hawaii license in good standing appropriate to the discipline.

Current Basic Life Support (BLS) certification.

Minimum two (2) years of experience in the SNF/ICF setting preferred.

Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).

Conduct evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.

Develop, implement, and carry out individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team.

Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life.

Educate residents, families, and caregivers regarding treatment plans and patient progress.

Document progress, changes in the plan of care, and discharges according to facility, state, federal, and payer requirements.

Must arrive the Thursday before the start to attend the employee health appointment on Friday.

Available to work Hawaii State and Federal Holidays as necessary.
