Physical Therapy Assistant

Provide physical therapy services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) to promote functional recovery and independence. Collaborate with the interdisciplinary team to develop and implement individualized treatment plans. Ensure compliance with state licensure, professional standards, and facility policies.

Responsibilities:

• Perform evaluations, treatments, and reassessments in accordance with licensure and scope of practice.
• Develop, implement, and modify personalized therapy plans in collaboration with supervising therapists and medical staff.
• Deliver evidence-based therapeutic interventions to improve mobility, strength, and overall function.
• Educate residents, families, and caregivers about treatment goals and progress.
• Complete accurate and timely documentation of clinical services, progress notes, and discharge reports.
• Supervise therapy assistants when applicable and ensure quality standards are maintained.

Qualifications:

• Hold current Hawaii State License as a Physical Therapy Assistant in good standing.
• Possess a valid Basic Life Support (BLS) certification.
• Minimum of two (2) years of relevant experience in SNF/ICF settings preferred.
• Demonstrate proficiency in electronic medical records systems such as Net Health Rehab Optima and Epic.
• Ability to work five days a week, including one weekend day, and adapt to floating shifts as needed.
