The SNF Rehab Team Leader plays a key role in coordinating daily rehabilitation operations and supporting a high-performing therapy team while ensuring residents receive quality, timely care. This position provides day-to-day leadership across PT, OT, and SLP services and serves as an important link between the rehabilitation team, facility leadership, nursing, and other interdisciplinary partners.

• Provide day-to-day leadership, guidance, and support to the facility’s PT, OT, PTA, COTA, and SLP rehabilitation staff.
• Coordinate daily therapy schedules, caseloads, coverage, and staffing needs to support effective department operations and continuity of care.
• Promote strong communication, collaboration, and teamwork within the rehabilitation department and across the interdisciplinary team.
• Support documentation, productivity, scheduling, quality, and regulatory standards, including maintaining an average productivity level of 50% while ensuring quality patient care.
• Assist with onboarding, orientation, training, mentoring, and ongoing support of rehabilitation staff; help address clinical or operational questions and escalamp concerns as appropriate.
• Participate in department and interdisciplinary meetings, quality improvement initiatives, and implementation of departmental goals and process improvements; help identify and address barriers affecting resident care, staff workflow, or department operations.

• Current professional license or certification in the applicable discipline:
  Physical Therapist (PT), Occupational Therapist (OT), Physical Therapist Assistant (PTA), Certified Occupational Therapy Assistant (COTA), or Speech-Language Pathologist (SLP).
• Minimum of 5 years of experience working in a skilled nursing facility (SNF) or similar post-acute rehabilitation setting.
• Demonstrated ability to communicate effectively and collaborate with an interdisciplinary team.
• Strong organizational, problem-solving, prioritization, and leadership skills.
• Ability to support day-to-day department operations, maintain professional boundaries, and appropriately escalamp concerns as appropriate.
• Must be able to perform clinical duties within the scope of the individual's professional license or certification.

OTA/PTA Range: $38.50 - $44.50/hourly
OT/PT/SLP Range: $50.00 - $62.00/hourly

Offers of employment will be contingent on successfully passing a pre‑employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen. The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.
