RN Health Coordinator - Field Based Oahu

About This Role

The RN Health Coordinator will work in the field on Oahu, HI, coordinating long-term care needs for patients and advocating for the best possible care available. This role is ideal for those who are dedicated to ensuring positive outcomes for patients.

  Up to 75% local travel required

• Assess, plan, and implement individualized care strategies directed toward the least restrictive level of care.
• Identify and initiate referrals for social service programs, including financial and psychosocial services.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate required health-related information to ensure quality coordinated care.
• Advocate for patients and families to ensure their needs and choices are represented.

• Current, unrestricted RN license in the state of Hawaii

• 2+ years of clinical experience
• Experience working directly or collaborating services for long-term care, home health, hospice, public health, or assisted living

• Current RN License in Hawaii required
• Driver's license and access to reliable transportation
• Current access or ability to obtain internet access via a landline
