Registered Nurse - LTC​/SNF

Job Description

Job Summary:

Provides comprehensive nursing care to residents in a long-term care or skilled nursing facility setting. Ensures the safety and well-being of patients through assessments, treatments, and medication administration. Collaborates with healthcare team members to deliver quality patient care.

Responsibilities:

• Perform assessments, treatments, and medication administration according to individual care plans.
• Notify MD/NP of pertinent clinical information and changes in patient condition.
• Assist residents with activities of daily living and provide specialty care such as G-tube, urinary catheter, wound vac, CPAP, and tracheostomy care.
• Admit and discharge residents, document care activities, and maintain accurate clinical records.
• Follow facility protocols and ensure compliance with healthcare regulations.

Qualifications:

• Hold an active Hawaii State RN license or obtain one upon acceptance of the position.
• Demonstrate experience in long-term care or skilled nursing settings.
• Be flexible to work varied 8-hour shifts including weekends and holidays.
• Possess strong clinical assessment and patient care skills.
• Capable of working independently and as part of a team.
