Registered Nurse - LTC​/SNF

Job Description

Job Summary:

Registered Nurse needed for LTC/SNF care within a long-term care setting on-site. The role focuses on patient assessments, direct care, medication administration, and coordination with providers to ensure timely, safe treatment.

Responsibilities:

• Perform ongoing patient assessments and document clinical findings.
• Administer medications and treatments according to established protocols.
• Notify the MD/NP of pertinent patient information and changes in condition.
• Coordinate patient admissions, discharges, and transitions of care.
• Provide ADL assistance and deliver individualized nursing support.
• Manage specialized care needs, including G-tube care, urinary catheter care, nebulized treatments, wound vac care, CPAP care, and tracheostomy care.

Qualifications:

• Hold an active State of HI Registered Nurse license.
• Obtain Hawaii licensure by the start of assignment if not currently in possession of an active HI license.
• Demonstrate competency in medication administration and clinical documentation.
• Show experience delivering skilled nursing care in LTC/SNF settings.
• Maintain current knowledge of respiratory and wound care practices (CPAP/trach, wound vac).
• Provide flexible availability for varied 8-hour shifts, including weekends and holidays.
