Registered Nurse (RN) Long Term Care (LTC) Position Overview

We are seeking a compassionate and skilled Registered Nurse (RN) with Long Term Care (LTC) experience to join our healthcare team on a contract basis. The RN will provide comprehensive nursing care to residents in a long-term care setting, ensuring high-quality patient outcomes through assessment, treatment, medication administration, and interdisciplinary collaboration.

• Perform comprehensive patient assessments and ongoing evaluations.
• Administer medications and treatments in accordance with physician orders and facility policies.
• Monitor patient conditions and promptly communicate significant changes to physicians, nurse practitioners, and other healthcare providers.
• Assist with admissions, discharges, and care planning activities.
• Provide support with activities of daily living (ADLs) as needed.
• Deliver specialized nursing care, including:
  • Gastrostomy tube (G-tube) care
  • Urinary catheter care
  • Nebulizer treatments
  • Wound vacuum (Wound Vac) therapy
  • CPAP management
  • Tracheostomy care
• Maintain accurate and timely documentation within the electronic medical record system.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Adhere to all facility policies, regulatory standards, and patient safety guidelines.

• Active Registered Nurse (RN) license in the State of Hawaii by assignment start date.
• Minimum of one (1) year of Long Term Care (LTC) nursing experience.
• Current BLS (Basic Life Support) certification.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient care abilities.
• Experience with electronic medical record systems; EPIC experience preferred.

• Varied 8-hour shifts.
• Weekend and holiday coverage required.
• Candidates must be flexible with scheduling, as hours and shift preferences are not guaranteed.

• Travelers without an active Hawaii RN license must apply for licensure upon offer acceptance and obtain licensure prior to the start of the assignment.
• Attendance at pre-employment health clearance and onboarding appointments is required before beginning the assignment.
