In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Boulder

Job Overview

Signify Health, part of CVS Health, is hiring a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role focuses on delivering comprehensive, preventive services and building trusted relationships with patients in their own homes.

• Conduct 45‑minute in‑home evaluations, including medication and medical history review, physical assessment, and any ordered diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each visit; no authority to prescribe medicine or order lab tests.
• Visit patients across the United States, traveling locally and within licensed states as needed.
• Perform virtual visits when appropriate.
• Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, bone density screening, and others.
• Maintain safety and professionalism in diverse home environments, including homes with pets, second‑hand smoke, and other potential allergens.

• Active, unrestricted license(s) in the coverage area; board certification required for NPs and PAs.
• Multi‑state licenses preferred.
• Anticipated weekly hours: 29.5.
• Ability to travel to Boulder, Grand and surrounding areas.
• Valid credentials and strong interpersonal communication skills.
