Job Overview

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You will engage with people in the comfort of their own homes and help bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You will use an iPad and an intuitive clinical workflow; prescribing, ordering labs, or altering current treatment regimens are not required.

• Be flexible to travel locally and within licensed states as needed

• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

As an in‑home clinician, your primary workplace is the private residence of health plan members. Candidates must be able to safely perform duties in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants.

The typical pay range for this role is: $50.22 - $108.15 per hour. The actual base rate will depend on experience, education, geography, and other factors.
