Advanced Practice Nurse (Nurse Practitioner)

As an Advanced Practice Nurse (Nurse Practitioner), you will:

• Practice to full extend of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.
• Practice in Primary Care Service at the Maui CBOC, Lana'i and Moloka'i VA Clinics to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veteran Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.
• Independently manages a panel of patients with acute and chronic health conditions with an emphasis in health promotion and disease prevention using evidence-based practice.
• Develop and implement an individualized plan to achieve optimal population health and disease management.
• Demonstrate leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
• Order and interpret labs, tests, and diagnostic studies.
• Prescribe pharmacologic and non-pharmacologic interventions.
• Initiate referrals and consults to specialty services.
• Provide education to patients and their families.
• Provide coverage for Maui PCPs or other VAPIHCS PCPs as needed.
• Assist with unscheduled walk-ins.
• Coverage for Employee and Occupational Health tasks for all duties including but not limited to pre-employment physicals, evaluation of acute injuries, COVID testing, tracking, vaccine programs, etc.
• Assess and adjust the plan if care as needed to achieve optimal patient outcomes.

Pay:
Competitive salary, regular salary increases, potential for performance awards
