Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Work with families to link them to Family Support Services and other community resources.
- Review Family Support Applications and determine eligibility.
- Develop Individualized Family Support Plans (IFSP) with the family.
- Develop individualized budgets for services and goods approved on the IFSP.
- Monitor goods and services the family is receiving under the IFSP.
- Review eligibility annually.
- Develop IFSP reviews annually.
- Complete all necessary paperwork.
- Enter information into the WIS system.
- Advocate for the family when appropriate.
- Provide advice and support to the families as needed and requested, including being available to listen to problems and concerns as well as successes and gains.
- Access community resources, information, and referral services.
- Develop partnerships and community collaboration to increase resources for families.
- Conducts the following services, supports, care and treatment, including but not limited to:
- Overseeing the services, supports, care and treatment provided to individuals;
- Supervising the formulation of the individual service plan or individual recovery plan;
- Conducting diagnostic, behavioral, functional and educational assessments;
- Designing and writing behavior support plans;
- Implementing assessment, care and treatment activities as defined in professional practice acts; and
- Supervising high intensity services such as screening or evaluation, assessment, partial hospitalization, and ambulatory or residential crisis services.
Bachelor's degree in a social service, special education, behavioral health or related field.
2 years experience working with individuals with intellectual & developmental disabilities.
Family Support experience preferred
Required Knowledge & Skills- Knowledge of disabilities and community resources
- Ability to relate to families with diverse ethnic, economic, and cultural backgrounds and circumstances.
- Ability to organize and track services and necessary documentation for the program.
- Ability to assist parents/caregivers, and other family members identify their concerns, strengths and priorities.
- Ability to empower other individuals with disabilities to explore new options, resources, relationships, feelings, attitudes and rights.
- Ability to work with others in public and private service systems to negotiate a plan that meets the needs of the family
- Ability to communicate effectively, verbally and in writing, to maintain confidentiality, and to work independently under general supervision.
