Upperline Health seeks a Nurse Practitioner to provide clinic‑based endocrinology care primarily in our {location}. In collaboration with Upperline’s local physician team, APPs develop complex plans of care aligned with patients’ health status, goals, and values; provide diagnostics and treatments in clinic, home, or via telehealth; and educate patients and families on medication usage, diabetes technology, lifestyle modification, diet, nutrition, and crisis anticipation & prevention.
APPs also communicate medical information to physicians, other providers, and the interdisciplinary team to ensure integrated, patient‑centered care.
- Develop and implement clinical plans of care for adult patients with endocrine and metabolic conditions such as type 1 & type 2 diabetes, thyroid disorders, obesity, osteoporosis, and adrenal/pituitary disorders.
- Gather patient history, perform focused physical exams, and order/interpret relevant diagnostics (e.g., A1c, CMP, lipid panels, thyroid function tests, microalbumin urine, endocrine dynamic labs) in clinic, via telehealth, or at patients’ homes.
- Initiate and titrate diabetes and metabolic therapies, including insulin, GLP‑1/GIP agents, SGLT2 inhibitors, and other endocrine medications following scope and protocols.
- Review and interpret glucose data from SMBG/CGM downloads; adjust regimens and coach patients to improve time‑in‑range and reduce hypoglycemia risk.
- Support diabetes technology as applicable (e.g., CGM initiation/education, insulin pump support with supervising physician and device educators).
- Provide patient education on medication usage, side effects, sick day management, hypoglycemia recognition/treatment, diet & nutrition, physical activity, and behavior‑change strategies.
- Coordinate preventive care and complication screening with the interdisciplinary team (e.g., eye exams, foot care, renal monitoring, cardiovascular risk reduction).
- Collaborate with interdisciplinary team to address patient and family physical and psychosocial needs, including referrals to nutrition, pharmacy, and community resources.
- Communicate with Upperline clinical team members and community providers to coordinate optimal care and resources.
- Review patient records and care plans with local physicians in a timely manner, consistent with state regulations and company standards.
- Maintain patient medical records and documentation in compliance with state regulations and company standards.
- Complete accurate and timely documentation per risk‑adjustment standards.
- Participate in weekly interdisciplinary team meetings and scheduled education sessions.
- Provide telephonic support for escalated calls from triage nurses.
- Participate in continuing education required by the state Board of Nursing.
- Prescribe medication as permitted by the state Board of Nursing.
- Other related duties as deemed necessary.
- Master’s degree in nursing (required)
- Board Certification as an Adult, Adult‑Geriatric, or Family Nurse Practitioner (required)
- Current State Registered Nurse and Nurse Practitioner license (required)
- Basic Life Support (BLS) certification (required)
- At least 6 months of nurse practitioner experience in Primary Care/Internal Medicine or Endocrinology (required)
- Endocrinology experience preferred; strong diabetes management/metabolic disease experience highly preferred
