-6 months duration.
The Case Manager, RN is responsible for the interdisciplinary planning, coordination, implementation and evaluation of health care services to ensure appropriate utilization review and discharge planning.
- Formulates a plan of care to meet identified needs in the most appropriate setting at time of initial assessment and documents in medical records.
- Performs utilization review function within 24 business hours of patient admission to the hospital and documents same on internal worksheet.
- Interviews patient and/or family with risk criteria for education needs and any special discharge concerns, and continues to follow through discharge.
- Actively participates interdisciplinary rounds and provides information to staff regarding Inter Qual criteria, payor guidelines and identified variances in the care plan.
- Identifies issues that require patient education prior to discharge.
- Degree in Nursing – BSN preferred
- Valid NJ RN
- Minimum 2-3 years’ experience in Healthcare setting preferred
- Milliman Guidelines, evidence-based clinical practice, that help determine the preferred course of treatment in medical situations.
- Successfully completes initial competency verification for specialty area.
- Demonstrated people/interpersonal skills.
- Ability to communicate clearly in English, both orally and in writing.
- Ability to handle confidential information.
