Director of Nursing – Home Health & Hospice
The Director of Nursing (DON) is a senior clinical leader responsible for overseeing all nursing and clinical operations within the Home Health and Hospice programs. This role ensures the delivery of high‑quality, patient‑centered care while maintaining regulatory compliance, operational efficiency, and strong interdisciplinary collaboration. The DON provides strategic leadership, fosters a culture of clinical excellence, and supports the organization’s mission to provide exceptional care to patients and families in their homes and communities.
Key Responsibilities Clinical Leadership & Oversight- Provide overall direction for Home Health and Hospice clinical operations, ensuring compliant, compassionate, and high‑quality patient care.
- Maintain deep knowledge of applicable federal, state, and local regulations (CMS, Medicare Conditions of Participation, state licensing, OSHA, etc.).
- Develop, update, and enforce clinical policies, standards, and best practices.
- Lead quality assurance and performance improvement (QAPI) initiatives, using data to improve outcomes and patient satisfaction.
- Recruit, train, supervise, and mentor clinical staff including RNs, LPNs, CNAs, therapists, social workers, and other care team members.
- Conduct performance evaluations, establish development plans, and promote a culture of accountability, growth, and teamwork.
- Oversee nurse scheduling, competency assessments, and clinical orientation programs.
- Ensure adequate staffing levels to meet patient needs and regulatory requirements.
- Oversee daily clinical workflows including onboarding, admissions, care planning, documentation, and discharge processes.
- Collaborate with executive leadership to create and manage budgets, staffing plans, and resource allocation.
- Ensure accurate and timely documentation in accordance with regulatory standards and company expectations.
- Partner with billing and compliance teams to support clean claims, audit readiness, and accurate documentation.
- Oversee development and execution of individualized care plans.
- Act as clinical resource and escalation point for complex cases, patient or family concerns, and critical decision making.
- Ensure smooth coordination between interdisciplinary teams including physicians, therapists, aides, chaplains, and social workers.
- Promote continuity of care across Home Health and Hospice services.
- Ensure full compliance with CMS, state licensing bodies, accreditation agencies (e.g., CHAP, Joint Commission), and all governing regulations.
- Lead preparation and response for surveys, audits, and inspections.
- Identify risk areas and implement corrective actions proactively.
- Develop and monitor clinical quality metrics, including hospitalization rates, patient satisfaction, timeliness of visits, clinical outcomes, and care transitions.
- Lead QAPI meetings, root‑cause analyses, and improvement plans.
- Promote patient and staff safety initiatives and evidence‑based practices.
