We are seeking a compassionate, independent Home Health Registered Nurse (RN) to deliver high‐quality, patient‐centered care in the comfort of patients’ homes. The Home Health RN performs comprehensive assessments, develops and implements individualized plans of care, provides skilled nursing interventions, and partners closely with physicians and interdisciplinary team members to promote safety, independence, and optimal outcomes. This role emphasizes strong clinical judgment, effective communication, and meaningful patient and family education across the continuum of care.
To support focus on patients rather than screens, home health care teams utilize ambient AI documentation technology that assists with real‐time clinical documentation, helping reduce administrative burden while maintaining RN oversight, accuracy, and regulatory compliance.

The Home Health Registered Nurse (RN) plays a critical role in delivering skilled nursing care and comprehensive case management services to patients in their homes. In this role, the RN functions as both a clinician and a case manager—conducting holistic assessments, developing and managing individualized plans of care, coordinating services across the interdisciplinary team, and monitoring patient progress throughout the episode of care.
The Home Health RN serves as the primary point of clinical oversight, ensuring care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals, while supporting successful outcomes and transitions across the continuum of care.
** Pay Range:** $34.00 - $49.18/hour

* Employment Type:
** Full Time, 72 hours every 2 weeks.
*
* Schedule:
** Day shift, Typically 8 hour days. Every 4th weekend.
** Requirements:
** Graduate of accredited Registered Nursing Program required, Current Michigan RN License (or completed by end of orientation) required, Current BLS for the Healthcare Provider (or completed within 60 days of hire) required
** Preferred Experience/
Skills:
*** Recent hospital experience and/or previous nursing practice experience preferred.
* 2 years prior RN experience in desired specialty area preferred.
* Prior experience with electronic patient documentation system preferred.
* Prior experience with technology common to specialty area (i.e., cardiac monitors, IV pumps) preferred.
* Bilingual preferred.
