Position: Visiting Nurse – Home Care (RN/LPN)

Our Visiting Nurses Association is the Home Care Skilled Nursing Team for West‑Central areas of Michigan. We serve patients in the Holland and Zeeland regions and around the state. Experienced RNs and LPNs are encouraged to apply, along with recent graduates. The role is autonomous, requiring strong clinical judgment and critical thinking. Nurses visit 5‑7 patients per shift in patients’ homes, including locations from Ludington to Gun Lake.

Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the RN assesses, monitors, and implements treatment plans, preventing physiological complications related to health problems or medical treatment.

Under the direction of the RN/Mid‑level Provider/Physician and in collaboration with other members of the health care team, the LPN contributes to assessment, planning, implementation, and evaluation of patient care. The LPN informs patients and families about treatments, medications, and equipment, and documents care via the electronic patient record.

• Associate’s in Nursing required.
• Bachelor’s in Nursing required within the last 5 years.
• MI Registered Nurse (RN) license required.
• MI Driver’s License required.
• AHA or ARC Basic Life Support (BLS) required within 90 days.
