- Provide direct nursing care and implement individualized resident care plans
- Administer medications, treatments, and pain management interventions safely and accurately
- Monitor resident health status, including assessments, skin integrity, wound care, and vital signs
- Observe, assess, document, and promptly report changes in resident condition
- Execute physician orders accurately and within required time frames
- Maintain accurate electronic health record (EHR) documentation and clinical charting
- Ensure compliance with infection control protocols and all State and Federal healthcare regulations
- Respond effectively to urgent and emergent resident care situations using sound clinical judgment
- Communicate clearly and professionally with residents, families, physicians, and interdisciplinary care teams
- Lead, delegate, and support CNA and clinical support staff while fostering teamwork and accountability
- Assist with staffing coverage and provide unit support as needed
Deliver comprehensive shift reports to ensure continuity of care - Provide exceptional resident-centered customer service by exceeding resident and family expectations
- Float between units based on operational and resident care needs
- Communicate with professionalism, empathy, and cultural sensitivity
- Build positive, collaborative relationships with residents, families, and team members
- Demonstrate strong critical thinking, clinical judgment, and decision-making skills
- Remain organized, detail-oriented, and adaptable in a fast-paced healthcare setting
- Work effectively both independently and within a multidisciplinary nursing team
- Maintain confidentiality and professionalism at all times
- Are proficient with electronic documentation systems and healthcare technology
- Are committed to continuous professional development and quality care improvement
- High school diploma or GED required
- Graduate of an accredited Licensed Practical Nurse (LPN) nursing program
- Successful completion of State Board of Nursing licensure examination
- Previous LPN/RN nursing experience preferred, ideally in long‑term care, skilled nursing, rehabilitation, assisted living, or senior care.
- Experience Working With Geriatric Or Elderly Populations Preferred
- Must maintain all continuing education requirements to keep nursing licensure active and in good standing
- Participates in ongoing learning, professional development, and required in‑service training
- Current, active Michigan Board of Nursing License as a Licensed Practical Nurse (LPN)
This role is active and hands‑on. You must be able to:
- Stand, walk, bend, stoop, lift, push, and pull throughout the shift
- Assist residents with mobility and activities of daily living as needed
- Safely lift up to 50 pounds
- Operate mechanical lifts and clinical equipment safely and effectively
- Maintain visual acuity and physical stamina necessary to deliver quality resident care
