Theywillberesponsiblefortheadministration and execution of an Independent Living Plusprogram for youth ages 16-19.They will overseea
Group Home and multiple Independent Living Plus Host Homesin the Holland/Zeeland Area,manage staff, host families, and coach youth to achieve their goals and the requirements of the program.
The Independent Living Manager will ensure that the multiple contracts for each client are followed, thatdocumentationis thorough andtimely, and that ongoing communication with clients and all other pertinent parties occurs regularly.
This position is community-based,and local travel is required.
- Implements andmonitorsthe qualityofindividual, group, and/orfamily counseling according to specific treatment plans usingevidence-based practices.
- Performs face-to-face meetings with assigned clients, according to contract requirements.
- Provides education, life skills development, training, and supportregardingthe needs of the clients.
- Schedules,organizesand attends Family Team Meetings.
- Develops and revises treatment plans for assigned clients according to each client’s individual needs.
- Ensures 24/7 crisis phone coverage is alwaysmaintainedamong staff.
- Maintains weekly contacts as appropriate (i.e. phone calls, meetings, letters, etc.) with schools, DHHS, lead agency, and mental health agency, as applicable, to ensure that allclientsreceive the level of advocacy that is required to meet the needs (social, educational, physical, medical, emotional, and cognitive) of each child.
- Serves as a liaison and client advocate in case conferences with the court, funding source, referring/referral agencies,schools and employers.
- Completes all required documentation and communication(e.g.updated assessments/service plans, case notes, court reports,andutilizationreview reports) in a timely manner.
- Maintains thorough records to ensure billing and payments areaccurateand timely.
- Monitors medication, as applicable, and documents inMiSACWISand client’s file.
- Develop and supervise the Independent Living team of staff at Escape.
- Plans and organizes work to meet all requirements according to agency standards, licensingrequirementsand funding requirements.
- Maintains a flexible work scheduletosupportthe needs of the community-basedprogramwith excellence.
- Work with a team of leaders tofacilitatea safe environment for youth to build relationships and engage in personal and spiritual growth.
- Actively supervise and engage with youth with joy and professionalism while on Escape property and in vehicles.
- Prepares for andparticipatesin individual supervision, team meetings, and other agency meetings.
- Stays informed of current practice developments within the profession and uses educational and training opportunities to ensure continual professional competence.
- Participates in continuous quality improvement efforts.
- Performs all other duties as requiredor requested by the agency.
- Works with community agencies/resources to provide referrals,resources,and related services to clients.
Qualifications:
- 3+years of experience in a child welfare agency,a child caring institution, or in performing a child welfare function.
- Bachelor’s Degreeor Master’sDegreefrom an accredited college or university in asocial work or related human services field (e.g.Psychology, Social Work, Human Development, Special Education)
- 1+ years of experience working in or managing an Independent Living Plus program, preferred.
- Canworka flexible schedule including evenings and weekends as needed.
- Valid Michigan Driver’s License
- CPR certified or committed to be certified within 3 months.
- Completion of Pre-Service Institute(PSI) training required by the State of Michigan within 6 months(candidates who have already completed PSItraining preferred).
