• Compensation: $64.00–$72.00 per point based on qualifications, plus weekend differential

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, and balance testing.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the Physical Therapy care plan and collaborate with nursing in the multidisciplinary plan.
• Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations related to client care using a state‑of‑the‐art touch‑pad tablet.

• Current Massachusetts license as a Physical Therapist.
• Minimum of one year of recent clinical experience as a licensed Physical Therapist.
• Prior home‑health experience preferred but not required.
