Solaris Health Care Coconut Creek is seeking an MDS Coordinator with SNF MDS experience for a part-time, hourly role. The candidate will support the Director of Nursing, ensure accurate MDS/CAAs, and coordinate the RAI process in a Skilled Nursing Facility.
Responsibilities include managing the MDS, care plans, and interdisciplinary communication while maintaining compliance with federal/state regulations. A current RN/LPN license is required with CPR certification and prior SNF MDS experience
