The individual in this position acts as the primary consumer advocate, keeps abreast of progress, and coordinates all services delivered to the consumer and family internally and through external referral.
Essential Job Functions- Conduct Needs Assessment
- Develop Service Plans in collaboration with the consumer and family
- Conduct monthly Service Plan reviews
- Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure
- Conduct individual case management sessions, as needed
- Perform activities on behalf of the consumer and family, including attendance at external appointments (e.g., court appearances, etc.), as needed
- Provide referral and linkage for the consumer and family, as needed
- Prepare monthly Progress Reports, as required, for assigned consumers for courts, probation officers, caseworkers, etc.
- Coordinate consumer and family care with the treatment team and with external providers
- Responsible for the development of the discharge plan in coordination with other treatment team members
- Arrange transportation for consumers, as necessary
- Assure implementation of and adherence to the behavioral management system, including providing behavioral consequences, motivational incentives, and learning experiences for the consumer
- Participate in case conference

- Participate in meetings and attend trainings as required and requested
- Understands HIPAA, Professional Ethics, Consumer Rights and Confidentiality
- Participate in performance improvement activities

- Perform any other duties as assigned.
- Certification as a Certified Case Manager (CBHCM or CWCM) in good standing by an agency approved and recognized by the Florida Certification Board is preferred at time of hire;
Preference will be given to those candidates who are currently Certified;
- Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
- A minimum two (2) years of related experience providing case management and/or care coordination services preferred.
- Bilingual preferred.
- Valid driver’s license and ability to obtain and maintain West Care’s vehicle insurance is required;
- Must have familiarity of Broward county
- Must have availability to work a flexible schedule including evenings and weekends.
- Maintains confidential files and records.
- Ability to identify and describe key signs, symptoms, impact and manifestations of trauma
- Ability to prioritize and organize competing consumer and family needs
- Ability to use trauma informed care principles including ensuring consumer safety, preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing consumer choice, maximizing collaboration, and emphasizing empowerment
- Aware of the differences between trauma informed and trauma-specific services
- Demonstrates ability to work effectively with staff and populations
- Demonstrates respect for individual's values when conducting assessments, developing service plans, and providing services
- Knowledge and application of psychopathology theory
- Knowledge in the understanding of Motivational Interviewing;
