Liberty Medical is seeking an experienced and compassionate Adult
Nurse Practitioner (ANP) to join our outpatient medical practice in Holmdel, NJ
. This position is ideal for a provider who enjoys building meaningful relationships with older adults and providing comprehensive, patient-centered care in an outpatient setting.

The Nurse Practitioner will evaluate, diagnose, and manage patients with a focus on geriatric medicine, chronic disease management, preventive care, and overall wellness. The successful candidate will work collaboratively with physicians and the healthcare team to provide high-quality care while helping patients maintain their health, independence, and quality of life.

• No Wednesday hours

• Perform comprehensive medical evaluations and physical examinations.
• Diagnose and treat acute and chronic medical conditions.
• Provide ongoing management of common geriatric conditions and multiple chronic illnesses.
• Develop individualized treatment plans based on each patient's medical needs and goals.
• Manage and monitor medications, including medication reconciliation and appropriate adjustments.
• Provide preventive healthcare, screenings, and health education.
• Assess functional status, fall risk, cognitive concerns, and other age-related issues.
• Coordinate care with physicians, specialists, family members, and other healthcare professionals when appropriate.
• Order and interpret laboratory tests, diagnostic studies, and other appropriate evaluations.
• Educate patients and families regarding diagnoses, treatment plans, medications, and lifestyle modifications.
• Maintain accurate and timely electronic medical records.
• Participate in quality‑improvement initiatives and contribute

• Current APRN/Nurse Practitioner license in the State of New Jersey
  .
• Board certification as a Nurse Practitioner through an accredited national certifying organization.
• Geriatric, Adult‑Gerontology Primary Care, Family Nurse Practitioner, or closely related experience preferred.
• Experience caring for older adults and patients with multiple chronic conditions strongly preferred.
• DEA registration and appropriate prescribing privileges.
• Excellent clinical assessment, communication, and interpersonal skills.
• Ability to work independently while collaborating effectively with physicians and other members of the healthcare team.
• Strong organizational skills and attention to detail.
• Commitment to compassionate, patient-centered care.
