The Service Facilitator plays a vital role in the Comprehensive Community Services (CCS) Program
, partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate resources to promote independence and well-being.
Key responsibilities include conducting functional assessments, creating person-centered plans, facilitating team meetings, maintaining accurate documentation, and advocating for clients using trauma-informed, strengths-based approaches.
Qualifications include a bachelor’s degree in Social Work, Psychology, or related fields, with preferred experience working with vulnerable populations. Strong communication, organizational, and relationship-building skills are essential, along with the ability to manage multiple services independently. A valid driver’s license and reliable transportation are required.
