Faith Home Health & Hospice is seeking a full-time Occupational Therapist (OT) to join their team in Holton, KS. The ideal candidate will provide Occupational Therapy services in alignment with the agency’s values of excellence, compassion, integrity, respect, and dignity.

Qualifications:

Licensed Occupational Therapist in Kansas

Graduated from an accredited OT program

Minimum 2 years of clinical experience preferred

Home healthcare experience preferred

Valid driver’s license and reliable transportation

Responsibilities:

Evaluate patients and establish treatment plans

Perform initial evaluations and re-evaluations

Prepare clinical notes and coordinate care with physicians

Instruct patients and families on therapy programs

Participate in case reviews and quality assurance activities
