compassionate, motivated Case Manager

What You’ll DoAs a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include:
- Conducting
thoughtful, person-centered assessments
to understand each consumer’s strengths, needs, and goals - Developing
individualized care plans
that support independence and quality of life - Coordinating services and community resources to ensure
smooth, responsive care - Monitoring progress and adjusting plans to support the best possible outcomes
- Advocating
for consumer rights, needs, and preferences - Collaborating closely with a
supportive, mission-driven team
and community partners
You’re empathetic, organized, and driven by purpose. You bring:
- A
Bachelor’s degree
in Social Work or a related field - Experience in case management, human services, or working with older adult populations
- Knowledge of community resources and healthcare systems
- Strong communication skills and the ability to build trust with consumers and families
- Comfort with technology, including electronic health records
- A valid driver’s license and reliable transportation
- Ability to successfully complete required background checks (CORI, SORI, SAMs GSA, Mass Health Exclusion List, OIG)

After 6 months and satisfactory performance, enjoy 2 remote days and 3 in-office days each week

- Home visits are typically 30-60 minutes depending on the assessment and visit type. How would you ensure to remain respectful with the consumer and stick to the allotted time frame?
- This position involves managing a caseload of approximately 100 older adult consumers. Are you comfortable working in a high volume, fast paced case management role?
