Client Care Manager LPN

We are seeking a Client Care Manager( Licensed Practical Nurse) to join our team at Nova Leap Home Health. This role is pivotal in ensuring that our clients receive the highest level of care and support. The Client Care Manager will oversee client interactions, manage care plans, and lead a team dedicated to enhancing the quality of life for our clients.

Responsibilities

• Oversee daily operations of the Client care department, ensuring high-quality service delivery.
• Manage client assessments and develop personalized care plans in collaboration with healthcare professionals.
• Lead and mentor customer care staff, fostering a supportive and effective team environment.
• Address client concerns and feedback promptly to enhance satisfaction and service quality.
• Ensure compliance with HIPAA regulations and maintain confidentiality of client information. Coordinate training sessions for staff on best practices in Alzheimer's and Dementia care. Requirements
• Proven experience in a customer service or management role within the healthcare industry.
• Strong skills in patient assessment and understanding of care planning processes.
• Excellent communication skills, both verbal and written, for effective interaction with clients and staff.
• Knowledge of HIPAA regulations is essential for maintaining patient confidentiality.
• Ability to lead a team with compassion while managing operational tasks efficiently
