Senior Care Options Supervisor

Salary:$53,000 – $57,000 per year
Job Type:Full-time

Schedule:

Monday–Friday, 8:00 AM–4:00 PM or 9:00 AM–5:00 PM (Typically new hires start with 9a-5p schedule)

Work Location:

Hybrid (up to 1 remote day per week after successful completion of the 6-month introductory period)

Access Care Partners is seeking an experienced, compassionate, and motivated

Senior Care Options (SCO) Supervisor to lead our team of Geriatric Support Service Coordinators (GSSCs). This leadership role is ideal for someone who is passionate about serving older adults, mentoring staff, and ensuring the delivery of high-quality, person-centered care.

As the SCO Supervisor, you will play a vital role in supporting care coordination services that help elders remain safe, healthy, and independent in their communities. You’ll work closely with interdisciplinary teams, healthcare providers, community partners, and agency leadership to ensure excellent outcomes for program participants.

What You’ll Do
  • Recruit, train, supervise, and support a team of Geriatric Support Service Coordinators (GSSCs) and Care Managers.
  • Provide coaching, performance management, and guidance on SCO policies, regulations, and best practices.
  • Review participant records, service plans, and documentation to ensure quality, compliance, and cost-effective service delivery.
  • Assist the SCO Director with program implementation, policy updates, and continuous quality improvement initiatives.
  • Support staff with complex cases, including occasional home visits, and provide backup coverage as needed.
  • Collaborate with interdisciplinary teams, healthcare providers, and community partners to develop person-centered care plans.
  • Monitor program performance and maintain accurate reporting and communication with leadership.
  • Build and maintain strong relationships with providers, primary care teams, and community organizations.
What We’re Looking For

Required Qualifications
  • 6–8 years of experience in aging services, care management, case management, healthcare, human services, or a related field.
  • Bachelor’s degree in Human Services, Social Work, Healthcare Administration, or a related field.
  • Strong leadership, coaching, and team development skills.
  • Excellent verbal and written communication abilities.
  • Strong organizational, problem-solving, and time-management skills.
  • Ability to work independently while managing multiple priorities.
  • Proficiency with Google Workspace and Microsoft Office applications.
  • Ability to learn and navigate multiple databases and software platforms.
  • Valid driver’s license and reliable vehicle available during working hours.
